Payment / Insurance:
Payment is due at the time of scheduled services. Payments may be made with cash, check, or credit card. Other important information about collection of fees will be covered in your consent form at your first appointment.
I do not accept insurance. However, if requested, I can provide you with a superbill to seek reimbursement from your insurance provider or other third party payer. Services may be covered in full or in part. Every plan is different so I recommend that you review your policy for receiving services from an out-of-network provider. Superbills should be requested in writing or via text message by clients or legal guardians. Please request them at least every 2-3 months. Feel free to call me if you have questions: 214-699-7762.
Cancellation Policy
Credit card information is required and will be kept in a
confidential file. If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you consent to have your credit card charged
for the full amount of the session.
